Any community physician or physician practice that serves pediatrics may participate in the program through one of the following:
1. Preferred Participation Method
Participation through an Child Health Advantage participating hospital network.
A physician or physician practice wishing to participate in Child Health Advantage through a participating Owner Hospital network must complete a Letter of Participation (LOP). The LOP provides the Child Health Advantage program with needed demographic information, distributor selection and physician office acceptance of program terms and conditions.
On the Letter of Participation (LOP), the physician or physician practice indicates their Owner Hospital network affiliation. The network indicated on the LOP will receive a copy of the LOP via email; the Owner Hospital network is then required to approve or deny the request. If the physician office is approved, CHCA will provide the LOP information to GlaxoSmithKline (GSK) and the selected distributor to activate the office within the Child Health Advantage Program.
2. Secondary Participation Method
Independent Child Health Advantage Participation.
A physician or physician practice wishing to participate independently in Child Health Advantage must complete a Letter of Participation as well as a Group Purchasing Agreement (GPA). This method of participation is for physicians and physician offices who are either denied network affiliation status or unable to find a network in their region.
The LOP provides demographic information, distributor selection and physician office acceptance of program terms and conditions. The GPA is a contract outlining the terms and conditions between CHCA and the physician office. Please contact the Child Health Advantage Program for a Group Purchasing Agreement.
Once the LOP and GPA are received in completed form, CHCA will provide the information to GSK and the selected distributor to activate the office within the Child Health Advantage Program.
